Getting Started with Autism Therapy at Rise Autism

We’re honored that you’re considering Rise Autism as a partner in your child’s growth journey. Before beginning services, please review our mandatory enrollment requirements and process below.

Enrollment Requirements

To enroll your child in our ABA or related therapy programs at Rise Autism, the following are required:

  1. Autism Spectrum Disorder (ASD) Diagnosis
    The child must have received a formal medical diagnosis of Autism Spectrum Disorder within the past 12 months.
  2. Active Insurance Coverage
    Families must have active insurance that includes coverage for ABA, EIBI, EIDBI, or other autism-related therapies. For specific accepted insurance providers, please contact our intake team or review our insurance information page.
  3. Minimum Commitment Period
    We require a minimum 6-month commitment to our programs to ensure consistency, continuity of care, and measurable outcomes for your child.
  4. Parental Involvement
    Family participation is essential. This includes:
    • Regular parent meetings
    • Monthly family skills training
    • Quarterly treatment plan reviews
    • Participation in Parent Support Groups and learning events
  5. Transportation Arrangements
    Parents/guardians are responsible for arranging transportation to and from our center. If you need help identifying transportation options, we’re happy to assist.
  6. Dietary Guidelines (If Applicable)
    If your child requires a specific diet while at the center (e.g., gluten-free, casein-free, or peanut-free), please inform our staff in advance so accommodations can be made.

Our Enrollment Process

To begin enrollment at Rise Autism, please follow these steps:

  1. Download and Complete the Application Form
    Visit our Forms & Documents Page to download the application form.
  2. Submit Required Documents
    Along with your completed application, please include:
    • A copy of your child’s ASD diagnosis
    • Insurance card (front and back)
    • Any additional medical documents or evaluations
  3. Insurance Verification
    Our team will contact your insurance provider to confirm eligibility and benefit coverage for therapy services.
  4. Schedule Intake Interview & Tour
    Once documents are reviewed, our Intake Coordinator will contact you to schedule:
    • An Intake Interview with our Clinical Team
    • A guided tour of our Minneapolis therapy center
  5. Waitlist Placement (If Applicable)
    After the intake interview, your child may be placed on our waitlist based on Clinical Team recommendations and current program availability.
